Medication Management Screening

Older persons will have their medications screened to identify medication-related problems and errors that may endanger the lives who participate in health promotion programs.

This service is provided by trained staff utilizing a computer system and appropriate data collection materials working in conjunction with a pharmacist or other medication expert.  The program consists of individualized screening, assessment, and alert processes to identify medication problems. 

Computerized screening and pharmacist review can help:

  • prevent falls;
  • dizziness;
  • confusion; and
  • other medication-related problems for older persons living at home.

Providers may also provide medication screening at health fairs throughout CIAA’s six-county geographic service area.
