Case Coordination Unit CIAA/Mature Solutions Community Care Program
If you are a person 60 years of age or older, or a Caregiver taking care of someone who is at least 60 or older, who is having difficulties with everyday tasks around the house, such as preparing meals, keeping the house clean, bathing, grooming, or finding a ride to doctors’ appointments, then you may be in need of Care Coordination Services. Care Coordination Services are designed to assist seniors to continue to live in the community for as long as possible by providing them with the necessary assistance to do so.
If you feel that you could benefit from these services, or know someone who could benefit from these services, then contact the Central Illinois Agency on Aging, so that we can assist you in finding the help that you need to remain in your home living as independently as possible. Then Central Illinois Agency on Aging can refer you to one of three Case Coordination Units; that provide Care Coordination Services to our six county service areas of Fulton, Peoria, Marshall, Stark, Tazewell, and Woodford Counties, and a care coordinator will come to your home to determine what needs that you have, and with your help develop a plan to address those needs. The Community Care Program currently offers In Home Services/Homemaker, Emergency Home Response System (EHRS), Adult Day Services, Money Management, and assessments for the home delivered meal program.

Pathways to Community Living
Transition Care Coordinators help eligible individuals in Nursing Homes or Long term facilities move back/transition into the community.
Mature Solutions II-Geriatric Care Management Program/Private Pay
A Geriatric Care Manager (GCM) is a health and human services professional, such as a nurse or social worker or other professional with geriatric experience related to aging and senior care issues. GCMs assist and advocate with older adults and their families to plan for and implement ways to allow for independence, safety, and comfort to remain in their home.
Veteran’s Independent Program (VIP)
Home and Community Based Services available to Veterans (must be referred by the Veterans Administration). Veteran Directed Care.
